| Please remember that all provider directories are updated frequently and are subject to change. When dental services are required, simply call the participating dentist you selected, schedule an appointment and present your membership ID card at the dental office. |

| In order to begin your search for a participating dentist, you must first know the name of your dental plan. The plan name is located on your membership ID card on line 2 directly under the member's name. Once you have located your dental plan name, reference the list below to determine the provider directory associated with your plan: |
